Sparks Family Medicine utilizes a holistic perspective incorporating Integrative and Functional Medicine to address the connectivity of physical and emotional issues, including a process called ATP Reset to evaluate, mitigate and manage exposures, stressors and trauma not often addressed by traditional medicine. Sparks Family Medicine requires new patients to complete an application to make sure that patient needs and expectations are aligned with the practice’s approach. If you are interested in becoming a patient of Sparks Family Medicine, please choose from one of the two programs below and complete the application for that program.

FM Program 1

$75 per month

  • Explore and Maintain Wellness.
  • Evaluation and management of health issues.
  • Access to the FM Platform.
  • “Whole person” wellness.
  • Routine and limited specialty labs.
  • Referrals, prescriptions and medical forms as appropriate.
  • $75 cash pay price for most office visits and telemedicine visits if not covered by insurance.

$125 per month

  • Address and Prevent Chronic Symptoms.
  • Evaluation and management of chronic issues.
  • Application of the FM Platform.
  • “Whole person” wellness.
  • Routine and specialty labs.
  • Referrals, prescriptions and medical forms as appropriate.
  • $75 cash pay price for most office visits and telemedicine visits if not covered by insurance.
