All of the patient information for Dr. Sparks' services prior to 2013, including phone numbers, addresses and medical charts, are the property of Amir Bacchus, MD., dba Healthcare Partners Medical Group.
Patients should contact Amir Bacchus, MD, dba HealthCare Partners Medical Group for all patient records for patient visits prior to 2013. To request that your medical records be provided to Sparks Family Medicine, LTD, please use the Medical Records Release #1(Click Here).
Enrolled in a Medicare Advantage plan such as United Medicare Complete or Humana Gold for the 2013 calendar year with the expectation that Dr. Amy Sparks could be your provider or;
Have experienced difficulty in continuing your care with Dr. Sparks in her transition to her new practice, including obtaining a copy of your medical records,
