Accepted Health Insurance
The providers at our office are credentialed with participating health plans through Sparks Family Medicine or Dr. Amy Sparks. If your provider, Dr. Amy Sparks or Sparks Family Medicine are not listed in your provider manual, our office does not participate with your health insurance plan.
As there are a multitude of health insurance plans for each health insurance company and plans are often contracted through third parties, we do not know all of the plans for which we may or may not be a participated provider. Please contact your insurance plan to determine if we are a participating provider before your first visit and upon changes to your insurance coverage.
